Health Risk Assessment (HRA)
We encourage you to complete a Health Risk Assessment (HRA) Questionnaire within 90 days of effective date of coverage and annually thereafter. This is a tool used by your care team to help determine possible health risk factors you may be facing, which gives the team the opportunity to connect you with resources and promote education, coaching, and wellness activities based on your specific needs. Cardiac and Pulmonary Rehabilitation
Cardiac and Pulmonary Rehabilitation (or cardiopulmonary rehabilitation) are supervised exercise programs that involve education, nutritional counseling, stress management, smoking cessation and other lifestyle changes designed to prevent or reverse the progression of cardiac and pulmonary diseases.
The Diabetes Self-Management Education and Support program includes a plan designed to help meet your diabetes needs, group or one-on-one classes, up-to-date diabetes information, discussion of nutrition facts, carbohydrate counting and the plate method and individualized meal planning with a dietitian.
If you have an upcoming joint replacement, Joint Camp provides you with the resources you need to ensure your surgery and rehabilitation are positive experiences. Experts will provide you with a comprehensive education detailing what to expect each step of the way.
