How to Enroll
Phelps Health Medicare Advantage
To enroll in a Medicare Advantage plan, you must first be eligible for and enrolled in Medicare Parts A and B, as well as live within our service area. Standard annual enrollment lasts from October 15th to December 7th.

Call
You can enroll in a Medicare Advantage plan over the phone by calling our toll-free line at 1 (855) 752-3795 (TTY: 711) and speaking to one of our licensed representatives.
Our phone lines are open 8 am to 8 pm, seven days a week from October 1st to March 31st, and Monday through Friday from April 1st to September 31st.
To enroll by mail, you’ll need an Enrollment Kit from Phelps Health Medicare Advantage. You can download one from our website using the button below, or request a physical copy be sent to your address.
If you choose to complete a physical form, it must be submitted to Phelps Health Medicare Advantage no later than December 7th for your enrollment to be accepted for the next calendar year.

The Phelps Health Medicare Advantage plan is available in Phelps, Pulaski, Maries, Dent and Crawford counties.
